Per la cura del tumore alla mammella siamo agli albori di una rivoluzione: terapie che “rieducano” il sistema immunitario a riconoscere il cancro e distruggerlo. In pratica attivando e accelerando le difese naturali dell'organismo, che vengono depresse dalle stesse cellule cancerose. È l'oncoimmunoterapia, che dopo essersi dimostrata molto efficace contro i melanomi e ultimamente anche il cancro al polmone, ora viene studiata anche per quello al seno. 
L'oncoimmunoterapia sembra particolarmente promettente proprio verso quei tumori al seno più aggressivi: gli HER2+ e i Tripli negativi. E non solo: sono proprio i tumori più mutati o capaci di mutare – quindi i più difficili – quelli che meglio vengono riconosciuti dal sistema immunitario, se adeguatamente aiutato dalle nuove terapie. Queste nuove terapie, per ora sperimentali, agiscono non tanto stimolando il sistema immunitario (per evitare il rischio di reazioni autoimmuni) ma depotenziando il freno che il sistema immunitario stesso si impone: in pratica quel recettore PD1 che viene utilizzato dai tumori per evadere la sorveglianza immunitaria. La cosa straordinaria è che la terapia immune è duratura nel tempo, il sistema immunitario diventa capace di controllare il tumore molto a lungo. Finalmente si può cominciare a parlare di guarigione.
Le possibilità dell'immunoterapia contro il cancro al seno sono state provate da due importanti studi. Il primo, presentato lo scorso dicembre, da ricercatori americani, israeliani e belgi, prevedeva l'uso pembrolizumab in 32 pazienti con tumori al seno triplo negativi in stato avanzato. Il secondo, presentato lo scorso aprile da ricercatori americani e francesi, usava una sostanza detta MPDL3280A in 54 pazienti con tumori al seno triplo negativi metastatici. Nel primo caso si è avuto una alla risposta alla terapia nel 18,5% dei casi; nel secondo, del 19%. Percentuali che non sono basse se si considera che sono i primissimi tentativi. Al momento sono in corso numerosi altri studi in vari istituti di ricerca. Nel convegno di Padova si è discusso delle prime impressioni sui risultati che stanno emergendo, sulle nuove vie da percorrere.
Negli ultimi dieci anni la ricerca contro il cancro ha puntato moltissimo sui cosiddetti farmaci “intelligenti” o “farmaci ‘target”, diretti cioè su bersagli molecolari presenti nelle cellule tumorali con l'obiettivo di modificarne alcune proprietà quali la capacità di replicarsi, di non morire nonostante i danni indotti dalla chemioterapia e radioterapia, di diffondersi ad altri organi. Questa via ha prodotto risultati significativi che hanno consentito in molti casi di aumentare la probabilità di guarire, soprattutto per alcuni tipi di tumore mammario quali i tumori a recettori ormonali positivi e i tumori HER2 positivi. Sono però emersi anche i limiti di queste terapie a causa della capacità del cancro di mutare continuamente e quindi di diventate insensibile agli stessi farmaci che funzionavano poco tempo prima. Per cui adesso si sta provando combattere i tumori utilizzando (oltre ai farmaci target) sostanze che attivano le stesse difese immunitarie dell'organismo. 
Il ragionamento è questo: se il cancro si è sviluppato è perché il sistema immunitario non ha funzionato e non funziona bene, in particolare perché è poco capace di riconoscere le cellule cancerose. Allora la nuova via terapeutica consiste nell'aumentare – con nuove sostanze – la capacità di riconoscere le cellule cancerose da parte dei linfociti e allo stesso tempo incrementarne l'efficienza nel distruggerle. Il primo tipo di tumore trattato così è stato il melanoma, con percentuali di successo molto alte (oltre il 60% di casi di regressione del tumore). Ottimi risultati si stanno ottenendo anche in quello del polmone. E adesso si sta mettendo a punto la terapia immunologica anche per il tumore al seno. Tutti i ricercatori sono concordi sul fatto che l'immunoterapia è il futuro dell'oncologia.

For the treatment of breast cancer we are at the dawn of a revolution: therapies that "re-educate" the immune system to recognize cancer and destroy it. These new therapies, for now experimental, act not as stimulating the immune system (to avoid the risk of autoimmune reactions) but weakening the brake that the immune system itself imposes itself: the PD1 receptor in practice that is used by tumors to evade surveillance immune. The extraordinary thing is that the immune therapy is durable over time, the immune system becomes able to control the cancer for a long time. Finally you can start talking about healing.
The possibilities of immunotherapy against breast cancer were tested by two major studies. The first, presented last December by American researchers, Israelis and Belgians, involved the use pembrolizumab in 32 patients with triple-negative breast cancers at an advanced stage. The second, presented last April by American researchers and French, used a substance called MPDL3280A in 54 patients with tumors metastatic triple negative breast. In the first case there has been a response to therapy in 18.5% of cases; in the second, of 19%. Percentages that are not low when you consider that are the first few attempts. There are several other ongoing studies in various research institutions. In Padua conference he discussed the results of the first impressions that are emerging, new ways forward.
In the last ten years, the cancer research has focused a great deal on so-called "smart drugs" or "drugs 'target', ie on direct molecular targets present in the tumor cells in order to modify certain properties such as the ability to replicate, not to die despite the damage caused by chemotherapy and radiotherapy, spreading to other organs. This way has produced significant results that have allowed in many cases to increase the probability of cure, especially for certain types of breast cancer such as cancer in hormone receptor-positive and positive HER2 tumors. They are also, however, showed the limitations of these therapies due to the cancer's ability to mutate constantly and thus become insensitive to the same drugs that worked recently. So now you're trying to fight tumors using (in addition to target drugs) substances that trigger the same immune defenses.
The reasoning is this: If the cancer has developed is because the immune system does not work and does not work well, especially because it is just able to recognize cancer cells. Then the new therapeutic way is to increase - with new substances - the ability to recognize the cancerous cells from the lymphocytes, and at the same time increase its efficiency in destroying them. The first type of cancer being treated so it was melanoma, with very high success rates (over 60% of tumor regression cases). Excellent results are obtained also in the lung. And now it is working on the immunological therapy for breast cancer. All researchers agree on the fact that immunotherapy is the future of oncology.
