A Milano la prima riparazione dei bronchi con cellule staminali.

L’Italia si è aggiudicata un altro primato: in un quarantenne colpito da un tumore polmonare è stata ottenuta la completa guarigione di una fistola bronchiale.
Lo studio, pubblicato sul New England Journal of Medicine, è stato condotto dall’Ieo (Istituto Europeo di Oncologia) con l’ateneo Statale e la Cell Factory della Fondazione Policlinico di Milano.
Si tratta del “primo caso mai realizzato di riparazione del tessuto bronchiale con staminali – spiegano i medici – che decreta in modo definitivo il passaggio dal laboratorio alla clinica di queste cellule studiate ovunque nel mondo per il loro potere di rigenerarsi nei tessuti in cui sono trasferite”.
Di seguito la spiegazione tecnica di quanto realizzato: “Abbiamo prelevato cellule staminali adulte mesenchimali dal midollo osseo del paziente – illustra Petrella – un giovane di 42 anni sottoposto all’asportazione del polmone destro per mesotelioma pleurico”, tumore noto alle cronache per i suoi legami con l’esposizione alle fibre d’amianto. “Le abbiamo espanse e poi inoculate tramite una metodica mininvasiva, la broncoscopia flessibile, nell’area del bronco dove si era creata una fistola post-chirurgica, una sorta di ferita aperta tra il bronco e il cavo pleurico, dovuta alla mancata cicatrizzazione fisiologica che normalmente avviene dopo la chirurgia. La metodica si è rivelata efficace nello stimolare la cicatrizzazione del bronco, evitando così altri interventi invalidanti. Oggi, a 8 mesi dal trapianto di staminali, il paziente sta bene e non ha avuto recidive”.

Per quanto a oggi conosciamo sulle cellule staminali mesenchimali – continua Petrella – sappiamo che sono in grado di migrare ed attecchire nelle aree di infiammazione e di danno ai tessuti. Una volta impiantate nel sito bersaglio da curare, nel nostro caso la fistola bronchiale, le staminali mesenchimali hanno la capacità di instaurare un contatto con il microambiente cellulare circostante, fenomeno definito in termini tecnici “cross-talk”, che consente un processo di riparazione e/o rigenerazione, con graduale ripristino delle funzioni danneggiate”.
Fino ad oggi la chirurgia del settore  è intervenuta su pazienti già debilitati  a seguito della chirurgia con tecniche invalidanti che non sempre regalano una certa qualità della vita. Avere dimostrato che le cellule staminali adulte possono determinare una riparazione naturale, oltre che un contributo notevole nelle tecniche di trapianto del tronco e della trachea è una vittoria senza precedenti, considerando che il trapianto con cellule staminali non crea rigetto.
Ma questo è solo un punto di partenza considerando che il nuovo obiettivo dei ricercatori è quello di affermare tale innovazione scientifica nell’ambito della pratica clinica.
Non sarà un iter breve perché serviranno ulteriori approfondimenti e protocolli sperimentali prima che la pratica prenda piede, diventando un modello da seguire e con l’intenzione di espandere la tecnica appena utilizzata anche su altri distretti anatomici.

But this is only a starting point whereas the new goal of researchers is to say that scientific innovation in clinical practice.
It will not be a quick process because it will serve more insights and experimental protocols before the practice to take root and become a model to follow, and with plans to expand soon also used technique on other anatomical areas.
