Leggere “a braccia tese”, aumentare la luminosità dello schermo o la dimensione dei caratteri sono i primi “sintomi” che, ahinoi, stiamo invecchiando. Superati i 40 anni, infatti, la presbiopia (questo il nome tecnico del difetto di vista che corrisponde alla riduzione progressiva della capacità di mettere a fuoco gli oggetti da vicino) colpisce miliardi di persone in tutto il mondo (1,8 miliardi secondo recenti stime). La soluzione è quella di rassegnarsi agli occhiali da lettura – un cambiamento che alcune persone fanno davvero fatica ad accettare. Ma per loro oggi potrebbe esserci un’alternativa: negli Stati Uniti sta per essere immesso in commercio un collirio che, agendo sulla capacità della pupilla di contrarsi, permette di lasciare temporaneamente gli occhiali nel cassetto. Attenzione però, è pur sempre un farmaco.

Un collirio da lettura

Il nome commerciale è Vuity e si tratta di una soluzione oftalmica di pilocarpina HCl 1,25%, che agisce sfruttando la capacità naturale dell’occhio di far restringere la pupilla per compensare la perdita di elasticità del cristallino. Una goccia al giorno di questo collirio in ciascun occhio, riferisce l’azienda Allergan che l’ha sviluppato, aumenta la capacità delle persone presbiti di mettere a fuoco gli oggetti a distanze brevi ed intermedie. L’effetto dura al massimo 6 ore.

Vuity, che è un farmaco che necessita di prescrizione medica, è stato testato in due trial clinici di fase 3 (Gemini 1 e 2, che hanno coinvolto complessivamente 750 persone presbiti tra i 40 e i 55 anni), che ne hanno appurato la sicurezza e l’efficacia.

Occhiali addio?

Le gocce oculari sembrano funzionare molto bene per correggere la presbiopia prima dei 55 anni e hanno dato pochi effetti collaterali non gravi (arrossamento degli occhi e mal di testa sono stati i più comuni). Come accennato, però, l’effetto è temporaneo e non è detto che non possa dare qualche disturbo. Vuity, per stessa ammissione di Allergan, può dare problemi nella messa a fuoco quando si passa dall’osservare oggetti vicini a osservare oggetti lontani, oppure in condizioni di bassa luminosità. Per questo attività come guidare potrebbero essere più rischiose, sconsigliate se la vista non risulta ben chiara.

Ogni paziente, poi, dovrà valutare con il proprio medico se sia un’alternativa praticabile per il caso specifico: la presbiopia potrebbe essere troppo importante per essere corretta dal collirio oppure possono esserci condizioni concomitanti che ne precludono l’utilizzo.

Anche i costi, poi, non sono da sottovalutare. Al momento negli Stati Uniti la spesa mensile per una persona che volesse usare il farmaco ogni giorno è di 80 dollari (circa 71 euro)

Insomma, più che sostituire gli occhiali da lettura dovrebbe essere considerato una comodità una tantum.

In fact, after the age of 40, presbyopia (this is the technical name of the vision defect that corresponds to the progressive reduction of the ability to focus on objects closely) affects billions of people around the world (1.8 billion according to recent estimates ). The solution is to resign oneself to reading glasses - a change that some people really struggle to accept. But for them today there may be an alternative: in the United States, eye drops are about to be placed on the market which, by acting on the ability of the pupil to contract, allows them to temporarily leave their glasses in the drawer. But be careful, it is still a drug.

A reading eye drops

The trade name is Vuity and it is an ophthalmic solution of pilocarpine HCl 1.25%, which works by exploiting the natural ability of the eye to make the pupil narrow to compensate for the loss of elasticity of the lens. A drop a day of this eye drop in each eye, reports the Allergan company that developed it, increases the ability of presbyopic people to focus on objects at short and intermediate distances. The effect lasts a maximum of 6 hours.

Vuity, which is a prescription drug, has been tested in two phase 3 clinical trials (Gemini 1 and 2, involving a total of 750 presbyopic people between 40 and 55 years of age), which have verified its safety and effectiveness.

Goodbye glasses?

Eye drops appear to work very well for correcting presbyopia before age 55 and have few minor side effects (eye redness and headache were the most common). As mentioned, however, the effect is temporary and it is not certain that it cannot cause some disturbance. Vuity, by Allergan's own admission, can give you problems in focusing when you go from observing close objects to observing distant objects, or in low light conditions. For this activity such as driving could be more risky, not recommended if the sight is not very clear.

Each patient, then, will have to evaluate with their doctor whether it is a viable alternative for the specific case: presbyopia may be too important to be corrected by eye drops or there may be concomitant conditions that preclude its use.

The costs are also not to be underestimated. At the moment in the United States the monthly expense for a person who wants to use the drug every day is 80 dollars (about 71 euros)

In short, rather than replacing reading glasses it should be considered a one-off convenience.
